Greetings to all Prospective Families!


We look forward to welcoming you and your family to our school and community.


Our Lady of Perpetual Help Catholic School uses an on-line application process.  Once the online application and documents are submitted, you will be contacted to schedule an interview and screening for your child.  When the student is accepted and enrolled at Our Lady of Perpetual Help Catholic School, the student and the family are expected to abide by and uphold the mission, philosophy, rules and expectations of the school and staff as stated in the Student/Parent Handbook.  All new students are monitored to ensure that they are doing their best academically and behaviorally.  A probation period of one semester allows parents, teachers, and administrators to evaluate the student’s success. 


Age Requirements

Preschool:  Students must be 3 by September 1 and be toilet trained.

Pre-Kindergarten:  Students must be 4 by September 1

Transitional Kindergarten: Students must turn 5 by December 31.

Kindergarten:  Students must be five by September 1

Submit an Online Application

The following documents must be uploaded in the online application or submitted to the school office:

  • Copy of original birth certificate
  • Current immunization records
  • Copy of last year's and current report card
  • Copy of last standardized test scores (i.e. IOWA, AIMs)
  • Forward Letter of Recommendation to applicant's current school


Assessment Testing and Interviews

  • Assessments and interviews are not required for Preschool, Pre-Kindergarten, and Transitional Kindergarten.
  • Kindergarten and 1st grade applicants are required to complete an entrance assessment.
  • Assessments and interviews may be requested for 2nd through 8th grade applicants on a case by case basis.


Admission Notification

  • The admission file will be reviewed and admission decision will be conveyed to the parents.


Enrollment

  • An email will be sent to the applicant's family with instruction for completing the enrollment and FACTS tuition agreement.
  • If the applicant has not been accepted, a letter will be sent to the family.
